Session Description: During the required ongoing review of physicians, we sometimes identify issues that require review of cases(s), patient/staff complaints, and other patient care opportunities. We are constantly looking for those red flags, trends and individual instances that might affect patient safety. However, sometimes, a physician will resign and relocate before an investigation is even initiated. In this program we will discuss the challenges related to these situations and what decisions need to be made to close out a review or to continue to pursue a review in order to confirm findings. The physician may decide to reapply in the future and you need to be ready with substantiated information for consideration during credentialing.

Understand the importance of continuing with a review of red flags related to credentialing or peer review issues following a physicians resignation.
Identify the key elements and essential documentation needed for the physicians file that will confirm conclusions in findings from review of physician.
Assure documentation is available for future credentialing review and decisions if the physician reapplies to the organization in the future
